Room Reservation Form - must be filled out and returned to our Programs Director - Kim.  You can email her questions at programs@shalimar-umc.org

The following forms apply to both Children and Youth

Parental Permission and Medical Release Form - Mandatory prior to participation in any trip away from the church grounds.  Must be notarized.

Safe Sanctuary Policy - a description of the church's policy for protecting children and youth from abuse.  We require that all volunteers read and agree to this policy before working with either children or youth.

Volunteer Agreement - the document must be completed by all volunteers prior to working with children or youth.  It must be notarized and indicates that you agree to abide by out Safe Sanctuary Policy and agree to submit to a background check should the church choose to perform one.
